This notice applies to Bluebird TMS, a trade name of TMS Therapy Centers of Colorado.

We may use or disclose your health information as required by law; for public health activities; to report abuse, neglect, or domestic violence; for health oversight activities; in judicial and administrative proceedings; for law enforcement purposes; to avert a serious threat to health or safety; for workers' compensation; and for other purposes permitted or required by law.

Other uses and disclosures, including most uses of psychotherapy notes, marketing, and any sale of your health information, require your written authorization. You may revoke an authorization in writing at any time.

Your rights

You have the right to inspect and obtain a copy of your health information; to request a correction; to request confidential communications; to request restrictions on certain uses and disclosures; to receive an accounting of certain disclosures; to receive a paper copy of this notice; and to be notified of a breach of unsecured protected health information.

Our responsibilities

We are required by law to maintain the privacy and security of your protected health information, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect. We may change this notice, and changes will apply to information we already hold; the current notice will be posted on this page and available at our office.

Complaints

You may file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
